We agree goals, tasks, frequency and escalation pathways, with consent and clarity.

3) Start & review

We begin safely and review regularly, adjusting as needs change.

Frequently asked questions

Do I need a referral?

Not always. For some services, we may need confirmation of clinical directions, prescriptions, or details from the treating team.

Can you provide nursing and care support together?

Yes, where appropriate. We can coordinate nursing input alongside care staff supports as part of one plan.

How quickly can services start?

This depends on location, assessed needs and scheduling. We aim to start as soon as safely possible after consultation.

What if there’s a concern or a change?

We follow agreed escalation pathways and communicate updates in line with the person’s consent and plan.
